This new benefit has many advantages over the current short term disability benefit package offered at your hospital. Ensure your paycheck is protected through a disability benefit that will pay over and above PFML benefits, sick time, vacation time, and workers’ compensation. For this time only, all benefits will be offered on a Guaranteed Issue basis – No Medical Questions.

ALLSTATE SHORT TERM DISABILITY (NEW*)

  • Protect up to 60% of income at a maximum of $5,000 per month
  • Protects your income due to maternity leave, accidents, surgery, and sickness
  • Pays in addition to PFML, earned time, vacation time, or workers compensation
  • PFML benefits do not reduce if collecting individual disability benefits
  • Covers you “on-and-off the job” (pays 40% of benefit if collecting workers comp)
  • Plan is completely portable and can go with you if you leave current employment
  • Pre existing conditions apply for the first 12 months

ALLSTATE HOSPITAL INDEMNITY (NEW*)

  • Provides cash benefits direct to the policy owner for hospitalizations
  • Pays $1,000 for hospital admission
  • Pays $150 per day hospital confinement benefits (up to 31 days)
  • Pays additional $150 per day if confined to ICU (up to 10 days)
  • Family coverage is available for Spouse and Child for additional cost

  • Provides cash benefits direct to the policyowner for accidents or injuries
  • Coverage includes payouts for: ER visits, X-Ray, ambulance, fractures, stitches, concussion, dislocations, burns, physical therapy, accidental death and more.
  • Wellness Benefit payable each year for annual physical
  • Family coverage is available for Spouse and Child for additional cost
