ALLSTATE SHORT TERM DISABILITY
- Protect up to 60% of your income, up to $5,000 per month
- Covers income loss due to maternity leave, accidents, surgery, or sickness
- Pays in addition to PFML, earned time, vacation time, and workers’ comp
- PFML benefits are not reduced if collecting individual disability benefits
- Covers you on and off the job (pays 40% of benefit if also collecting workers’ comp)
- Portable coverage — take it with you if you leave your employer
- Pre-existing conditions apply for the first 12 months
ALLSTATE HOSPITAL INDEMNITY
- Cash benefits paid directly to you for hospital stays
- $1,000 for each hospital admission
- $150 per day hospital confinement benefit (up to 31 days)
- Extra $150 per day for ICU confinement (up to 10 days)
- Family coverage available for spouse and children at an additional cost
ALLSTATE ACCIDENT COVERAGE
- Cash benefits paid directly to you for covered accidents or injuries
- Includes payouts for ER visits, X-rays, ambulance, fractures, stitches, concussion, dislocations, burns, physical therapy, accidental death, and more
